Physical & Health History Update Forms are currently available and can be printed out. The Physical and Health History Update Forms can be dropped off at the High School Main Office.

*NEW* Physicals should be maintained by the healthcare provider completing the physical exam (medical home). It should not be shared with BRHS. The last page “Medical Eligibility Form” is the ONLY form that should be submitted to the BRHS Health Office. The physical exam must be completed by a healthcare provider who is a licensed physician, advanced practice nurse, or physician assistant who has completed the Student-Athlete Cardiac Assessment Professional Development module.

Please make a copy of your physical or any other paperwork before submitting it for your records.   

The Physical and Health History Update Forms are still hard copy forms that need to be filled out and turned into the Nurses' Office. For a Physical or Health History Update to be approved by the first day of try-outs/practices it must be submitted to the Nurse's Office. The Physical Form and Health History Update Forms can be printed by clicking on the prior links. Physical Forms can be handed in at the High School Main Office Monday through Friday from 8:00am to 2:00pm. It is recommended that all candidates make a copy of their physical prior to handing it in. Physicals must be completed within 365 days prior to the start of the sport. All Physicals must completed on the NJ Department of Education Pre-Participation Physical Evaluation Form (PPE).

Athletic online registration for the fall, winter, and spring seasons through the PowerSchool Parent Portal will open on Monday, July 15, 2024, at 11:00 am.

No one may participate in try-outs/practices without a Physical that has been approved by the School Physician. 

The ImPACT Test should not be taken unless instructed to do so by the Athletic Training Staff. Your son or daughter will be informed by the Athletic Trainers if they are required to take the ImPACT Test. Please do not do so unless directed.
